Community Care Development Network

Founded in 2014, Community Care Development Network (CCDN) is a faith-based, 501(c)(3) nonprofit dedicated to strengthening and empowering families and communities in Jefferson County, Alabama. Rooted in faith and guided by a mission of compassion, we believe in uplifting families and individuals through practical support, education, and community resources. Our programs are designed to address not only immediate needs but also the underlying causes of poverty, promoting lasting, positive change.

At CCDN, we approach our work holistically, understanding that families and individuals thrive when given the tools to build resilience and achieve self-sufficiency. Our comprehensive services, grounded in the Strengthening Families Model and a focus on the Social Determinants of Health (SDH), are built to meet the unique needs of our community. We take pride in collaborating with local partners, community leaders, and volunteers to maximize our impact and create sustainable pathways for success.
